ITEMS NEEDED TO PROCESS YOUR APPLICATION:

  1. FILLED OUT APPLICATION

    1. Please do not sign or attach your photo ahead of time. You must appear in person.

  2. PHOTO

    1. There are strict guidelines concerning passport photos. Please get them taken at a facility equipped to take a proper photo.
    2. The nearest location to Deer Lodge is CVS Pharmacy in Anaconda. Otherwise Osco Drug, Walmart, and Walgreens can take photos.
    3. Photos must be taken within 6 months of submitting your application.

  3. CITIZENSHIP EVIDENCE

    1. One of the following is required:
      1. Old Passport
      2. Certified copy of Birth Certificate
        1. Must have parents’ names and an embossed seal from the issuing state.
      3. Original naturalization certificate
    2. This is sent in with your application and will be returned to you in a separate mailing.

  4. IDENTITY DOCUMENT

    1. Valid Driver’s License
      1. If not a Montana Driver’s License, then additional ID is required
    2. Previous Passport book or card
    3. Certificate of Naturalization with photo
    4. Certificate of Citizenship with photo
    5. Official US military I.D. card
    6. Government employee I.D. card
    7. Current (valid) foreign passport

  5. FEES

    1. Check or money order in correct amount made out to “US Department of State”
      1. If submitting multiple applications, separate checks are needed for each
    2. Execution/Processing fee in cash or check to “Passport Facility”
